Computer Consent Form
The Plainview-Old Bethpage Central School District recognizes in today's environment it is
necessary for select staff members to have access to computer resources such as hardware,
software, networks, e-mail, etc. and to be able to utilize the Internet. It is anticipated that staff
access to these resources will both expedite and enhance the performance of tasks associated
with a staff member’s assignments and position.
In order to utilize the district's computer resources and have access to the Internet it will be
necessary for you to sign the bottom of this memorandum and return it to the Personnel
Department. By signing below you are acknowledging that you have read this memorandum
and the attachments and that you agree to adhere to the requirements set forth in these
documents. If you do not have a signed form on file, your access rights will be suspended.
It should also be noted that by acknowledging agreement you will be allowed a broader range
of research ability than is allowed for students. While this will provide you greater ability to
find information it also requires that you use a greater level of caution in accessing Internet
sites. In consideration for the privilege of using the Plainview-Old Bethpage School District's
computers and related hardware and software including the district's Computer Network you
agree to release and discharge the Plainview-Old Bethpage Central School District, and its
respective officers, employees and agents, from any and all claims and liabilities arising out of
or resulting from any use, operation or inability to use them and further agrees to defend and
indemnify the District and hold the District harmless from and against all claims, proceedings,
damages, injuries, liabilities, losses, costs and expenses (including reasonable attorney's fees)
relating to any acts taken by the undersigned in connection with any use of the District's
computers and related hardware and software including the district's Computer Network.
